If a copy of the report is required, we recommend that referees compose their comments into a separate plain text document, then paste the text into the web form.

Comments may also be uploaded as word processor or PDF files using the option at the bottom of the form.

After submission, a copy of the submitted report will be sent to the email address entered on the form.
