Page 15411: Two errors appeared in the sentence beginning on line 10 from left bottom "to assign the two emission peaks at 493 and 516 nm to the phosphorescence emissions of f-PVK and p-PVK, respectively.^{13, 17}" The sentence should read as follows: "assign the two emission peaks at 493 and 516 nm to the phosphorescence emissions of p-PVK and f-PVK, respectively. ^{13, 17}"

Page 15411: The sequence of "f-PVK and p-PVK" on line 2 from left bottom was incorrect, the correction should be "p-PVK and f-PVK". In other words, the two decay lifetimes of 0.45 and 1.4 μs should correspond to the two phosphorescence emissions of p-PVK and f-PVK.¹³ And then the sentence was incorrect and should be ignored beginning on line 2 from left bottom "Since the f-PVK conformation is more rigid than the p-PVK conformation, the short lifetime (0.45 μs) should belong to the former."

Page 15412: A sentence was omitted from the legend to Fig. 3. The legend should read: (a) Diagram of the different formation mechanisms for the phosphorescence excited by PL and EL. (b) The temperature dependence of the EL spectrum for ITO/PEDOT/PVK (290 nm)/CsF/Ca/Al at ~10 μA current.
